Submitting a Demographic Request to Add an Address/Contact

Use the Demographic Add Request page to submit a request to add an address to your vendor record on file with the customer. The Demographic Add Request page has three areas:

To submit a request to add an address:

  1. On the Demographic Request Queue, click the button at the top of the Addresses grid. The Demographic Add Request page appears.

  2. In the General Information area, read the message broadcast by the customer, if available.

  3.  In the Request Information area, enter the following fields.

Description

Mailing Name

Type the mailing name for the vendor. You can enter up to 40 alphanumeric characters.

Address

Type the following information in the Address fields:

  • In the first three lines, type the street address for the vendor. You can enter three lines with a maximum of 40 alphanumeric characters per line.

  • On the fourth line, type the city, US Postal Service state abbreviation, and zip code. You can type a maximum of 40 alphanumeric characters in the City field , and a 5- or 9-digit zip code.

Request Comment

Optionally, type the reason for this request.

  1. If applicable, in the Contacts grid, click the button to add a contact to the demographic request. Click here for the steps required to add a contact record.

  2. If applicable, in the Request Attachments grid, click the button to attach a document to the demographic request. Click here for the steps required to attach a document.

  3. Click the radio button of your choice:

  4. E-mail me only on the final step of the approval process. to receive an e-mail when the request is approved (default).

  5. Do not e-mail me about this request. if you do not want to receive an e-mail when the request is approved.

  1. Click the button to submit the pending request for approval. The Demographic Request History Queue displays with the pending request displayed in the Request History grid.

Click the button to close the Demographic Add Request page without saving the request.
